Admission
Admission requirements:
Pick up the application from the office to fill out and submit.
An updated Universal Child Health Record along with an immunization record, and a special care plan, if applicable.
A copy of the Birth Certificate.
Registration Fee $300.
A written note with a doctor’s signature from your child’s physician regarding food allergy restrictions.
No child will be admitted to the center until the above-mentioned procedure and forms have been completed.
St. Elizabeth school and Child Care Center procedures ensure that personal information concerning students in the program is given only to the authorized individuals. Furthermore, no information with respect to the confidential record of a student will be released at any time without the written approval of the parents/guardians unless under court order.
